Community stabilization helps to prevent hospitalization or re-hospitalization by providing a stable environment with a high level of support services for individuals experiencing a severe decline in their mental and emotional wellbeing.

Services include but are not limited to: Mental health evaluations and assessments, clinical evaluations, individual and family counseling, 24-hour on-call support, assistance with accessibility of other services, education on relapse prevention, and connection to additional community resources.

Experiencing a severe decline in their mental and/or emotional well being

Transitioning to a step-down program from a higher level of care

At risk of a higher level of care and require short-term support
