Substance use case management services (SUCM) assists individuals in accessing and connecting to needed community resources, in order to increase independent and life skills.

Our case managers meet with clients at least 2-3 times per month, depending on the individual's current needs. Case managers educate and link clients to different resources based on their individual needs, including:

Case management activities include but are not limited to: Scheduling medical appointments, assistance with transportation, applying for social welfare services, employment/resumé building, GED assistance, community engagement, and relapse prevention planning
