Substance Use Case Management Services (SUCM)

Adults 21 and older
 

• Substance use case management services (SUCM) assists individuals in accessing and connecting to needed community resources, in order to increase independent and life skills.
   

• Our case managers meet with clients at least 2-3 times per month, depending on the individual's current needs. Case managers educate and link clients to different resources based on their individual needs, including:
  
  
  
  
  
   

• Case management activities include but are not limited to: Scheduling medical appointments, assistance with transportation, applying for social welfare services, employment/resumé building, GED assistance, community engagement, and relapse prevention planning
