
Substance Use Case Management
Working forward together to maintain sobriety
SUCM Case Managers perform anywhere from 2-3 case management activities per month, depending on the client's current needs and level of commitment to ensure compliance of activities/services.
SUCM services are open to residents and clients looking for some additional support during and even after discharge from ASAM levels 2.1 and 3.1. The additional supports can range from helping the clients link to community resources in order to gain independence, helping them with delegating/managing areas of basic living needs and life skills, such as but not limited to:

Identify and access resources such as SSI, SSDI, etc.

Managing care coordination with service providers, healthcare providers

Medication management/compliance
-
Employment skills and support
-
Treatment and Recovery Reviews

Nutrition/wellness exercises and self-care programs

Household management
-
Constructive use of leisure time
With some of the activities listed above, our Case Managers help link, connect, advocate, and help assist our clients with attaining skills to become self-sufficient and improve their independence.
Paying For This Service

Guidelines For Admission
The Medicaid eligible individual shall meet the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) diagnostic criteria for a substance use disorder and require assistance with accessing needed medical, psychiatric, psychological, social, educational, vocational, recovery, and other supports essential to meet the individual's basic needs.
Tobacco-related disorders or caffeine-related disorders and non substance-related disorders shall not be covered.
